Account Types

Children’s Health provides 3 kinds of MyChart accounts:

  • Proxy Account
    (for parents and legal guardians)

A proxy account allows a parent or legal guardian full access and control over the account for a child under the age of 18. A child may have more than one person designated as their proxy. Each proxy must have their own Children’s Health MyChart Patient Portal account.

Caregivers are not eligible for proxy accounts for children under the supervision of Child Protective Services.

    Request a proxy account

  • Minor Account
    (for patients age 14-17)

A patient aged 14 to 17 can have a minor account if their parent or legal guardian consents. The minor account and any associated proxy account will have access to the same MyChart features and all of the minor’s personal health records.

A proxy can request a minor account for their child in MyChart by navigating to Menu > Minor Account (14-17) > Parental/Guardian Consent and completing the Parental/Guardian Consent Access to MyChart form.

Children in foster care and their caregivers are not eligible for MyChart accounts.

  • Adult Patient Account
    (for patients age 18+)

A patient 18 years of age or older has their own account, and full access and control over their account. If a parent or legal guardian wishes to have access to their adult child’s account, the child can give proxy access in MyChart by Navigating to Menu > Sharing > Sharing Hub.

    Request an adult patient account

Note that children under the age of 14 cannot have their own accounts.

To protect the child’s health information, our Health Information Management Department must verify a parent or legal guardian’s identity before giving full proxy access. This may take up to 5 business days.

Until then, you can log in to the Children’s Health Patient Portal to view your child’s upcoming appointments, complete PreCheck-in tasks, or review COVID-19 and RSV lab results from an emergency room visit. These can be accessed from your child’s health record.

Need Help?

If you are not granted full proxy access within 5 business days of creating your Children’s Health MyChart account, or if you have issues creating your patient portal account, call our Health Information Management Department at 214-456-2503.
